Abakast-L, a combination of Abacavir (600 mg) and Lamivudine (300 mg), serves as an antiviral medication aimed at preventing HIV-1 and HIV-2 infections. It operates primarily by inhibiting the reverse transcriptase enzyme, essential for viral replication. Administered orally, Abakast-L terminates the chain reaction in the viral replication cycle, thereby hindering the progression of HIV.
Administration:
Abakast-L is taken orally, with a bioavailability of approximately 83%. Both Abacavir and Lamivudine exhibit mean to maximum serum concentrations within an hour of ingestion. Food intake does not significantly affect absorption. The drug effectively penetrates the cerebrospinal fluid, with a CSF to plasma AUC ratio ranging from 30-44%. Metabolism occurs mainly in the liver, with less than 2% of the dose excreted unchanged. Lamivudine’s mean clearance is approximately 0.32 L/kg/h, with renal clearance contributing to about 70%.
Dosages:
For adults (≥12 years), the recommended dosage is 600 mg once daily. Children under two years receive dosages based on weight (4 mg/kg twice daily). Those weighing 30 kg should take 300 mg twice daily.
Elimination:
The half-life of Abakast-L is around 1.5 hours, and elimination primarily occurs through urinary excretion of metabolites. Approximately 83% of the dose is excreted as a combination of metabolites and unchanged drug, with the remainder eliminated in the feces. Lamivudine’s half-life is extended, lasting around 10 hours in blood lymphocytes.
Side Effects:
Hypersensitivity reactions, occasionally fatal, may occur. Lamivudine use can lead to opportunistic infections and complications related to HIV infection. Symptoms indicating multi-organ involvement, fever, rashes, nausea, vomiting, diarrhea, fatigue, and paresthesia may manifest. Respiratory symptoms like sore throat or cough are possible.
